We would like to invite you to register your child/children to participate in this year's Lodi Adopt-A-Child Christmas Wish program for children 13 years old and younger living within the Lodi Unified School District. Please see the details below regarding your registration appointment time and documents you will need to bring with you.
When: Tuesday, October 25 between 9:00 a.m. - 1:00 p.m.
Where: 100 E. Pine St., Lodi
What to bring:
Proof of residence (utility bill, rental agreement, etc.)
Birth certificates for all children
Proof of income (pay stub or similar) - See income guidelines below
Toy wish (reasonable Items -please no laptops, gaming systems, iPads, or the like), children's clothing sizes and shoe sizes
If requesting a bicycle, please have an alternate toy wish as well as bikes may be in short supply.
Must meet income guidelines to qualify
Information regarding gift pick-up will be provided when you register.

If you are unable to print this application, copies will be available at the Lodi Adopt-A-Child offices at 100 E. Pine St., Lodi.
Please keep in mind that families that participate in the Lodi Adopt-A-Child Christmas Wish program agree not to participate in any other Christmas program offered by local organizations. This helps all organizations reach as many children as possible.
